MSU faculty must submit a formal [PDF] Request for Clinical Placement (228 KiB) and attach required documents as early as possible in the semester. CFE staff is available to assist faculty and students with facilitation of the [PDF] Criminal Background Check (182 KiB) process, and faculty may request presentations be made in class during the first two weeks of the semester. Contact the CFE Assistant Director in Armstrong Hall 301-I (extension 5716) to schedule a presentation.
Students requiring background checks for clinical experiences need to return the correct background form with a check to the instructor by the 2nd class meeting of the semester. Any late forms with checks should be turned into the Office of Clinical Experience located in 119 Armstrong Hall. All instructors need to submit a class roster with each batch of forms and checks making sure that the checks are payable to AP or Accountability Plus, and not MSU.
For students seeking placements on an individual basis, a [PDF] Student Placement Information Form (658 KiB) is required to facilitate placements according to schedule availability. All students must read the Clinical & Field Experience Policies and Procedures Guidelines prior to the start of any clinical experience in the public school setting and adhere to the guidelines throughout their experience. Faculty is reminded to submit [PDF] Clinical Class Profile forms (711 KiB) and post related evaluations to the university assessment system prior to the conclusion of each semester.
