Application for Licensure

Understanding the process

What you must know is that you must send ALL your documents for licensure to the University.  We then compile all the required supporting documents and send your entire application directly to PELSB. 

To apply you must first determine that you are ready to apply.

  • In order to qualify for licensure your degree has to be officially awarded or your all grades must be posted.   
  • You must have taken all of your licensure exams that correspond with your licensure. You must wait until you receive your Official Score report, whether you pass or fail. 

Once you are ready to apply, complete this form. Completing this form allows staff to review your results and determine your next steps for application. Once reviewed, you will receive an email within four working days.
